We are looking for a Social Worker to join the OHOW team! You will work for a small, team-oriented organization that values patient and staff satisfaction. This position is a pool Day shift position.

Core Responsibilities

Coordinates the overall interdisciplinary plan of care for patients, from admission to discharge.

• Creates plan for care across the continuum, integrating patient/family preferences and values. 
• Services as a resource to assess, evaluate, and recommend legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs. 
• Advocates for resources and removal of barriers. 
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation of health plan. 
• Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
• Assist patients with verification of payer benefits and eligibility. Educates them on their benefit information.

Requirements

• Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy. 
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
• Associate’s degree/Technical degree or 4 years of applicable cumulative job specific experience required.
• Customer Service skills

This position is located at the Orthopaedic Hospital of Wisconsin (OHOW). OHOW was established in 2001 as a joint venture between a group of orthopedic surgeons and Ascension Wisconsin.

OHOW focuses solely on the full-service treatment of orthopedic injuries and conditions. We provide surgery, inpatient overnight stays, laboratory services, diagnostic imaging and rehabilitation services.
